WELCOME LETTER TO PARENTS AND STUDENTS

Dear Parents and Students,

I want to take this opportunity to welcome the new students and their parents to our school and welcome back the students who attended last year and their parents. The faculty, staff and I are excited about working with you. We are glad you chose our school and are looking forward to having you on our team.

We want you to remember that our school is also your school and we welcome you into the activities of the school. We always welcome volunteers in the school. On a daily basis we need volunteers to read with students and to help monitor the cafeteria during breakfast and lunch. We encourage you to visit the school often and have conferences with your child's teacher.

The following items may be required to enroll your child in our school:

1. Completed enrollment forms.
2. Copy of original Birth Certificate (not hospital copy).
3. Immunization Certificate (schools must receive certificate within 5 days of enrollment).
4. TB Test Certificate.
5. Medical Examination Form (physical required for Preschool or Kindergarten)
6. Copy of Social Security Number.

The Department of Health Service requires a second dose of measles-mumps-rubella (MMR) vaccine for entering Preschool, Kindergarten or 6^th grade. Your child's immunization must also include Measles, Mumps, Hepatitis B, Haemophilis Influenza.
